How do you create an exam blueprint?
Define the purpose and content domain of the exam, list the learning outcomes it must cover, weight each content area by curricular emphasis and clinical importance, cross those weights with cognitive levels in a specification table, then allocate item counts per cell and commission items to fill them. Review and sign off before item writing starts.
Building a blueprint is a planning exercise that happens before item writing — not a retrospective sorting of whatever items exist. The logic mirrors the backward design that Wiggins & McTighe describe for curricula: start from the outcomes you need evidence about, then work back to the assessment that would produce that evidence.
Step by step
1. Define the purpose and the domain. State what decision the exam supports (progression, remediation, end-of-clerkship grading) and which part of the curriculum it samples. A blueprint for a summative end-of-year exam looks very different from one for a formative quiz.
2. List the content areas and outcomes. Pull these from course objectives or the program's competency framework, not from lecture titles. Hamdy (2006) recommends anchoring the blueprint in competencies and, for clinical exams, in patient problems and physician tasks rather than topics alone.
3. Assign weights. For each content area, decide its share of the exam. Common criteria are teaching time, importance to safe practice, and frequency of the problem in real clinical settings. Weights should be agreed by the exam committee, not set by a single author.
4. Add a second dimension. Cross content areas with cognitive levels — at minimum, recall versus application/analysis. Medical exams generally push most items toward application: clinical vignettes that require using knowledge, not reciting it.
5. Convert weights to item counts. Multiply each cell's percentage by the total test length. Be realistic about testing time; if a cell rounds to fewer than one item on this form, plan to cover it across forms over the year rather than forcing it in.
6. Commission and tag items. Give writers their cell assignments (content area, cognitive level, sometimes item format), and tag each item in your question bank against the blueprint so coverage can be audited automatically. 7. Review, sign off, and archive. The committee checks the assembled exam against the blueprint, documents deviations, and files the blueprint as part of the exam's validity evidence.
After the exam
The blueprint is also the frame for interpreting results.
